Former registrants must submit the completed application form to the College along with the following:
- Application fee of $125 (payable by cheque or money order made out to the College of Midwives of Ontario, or by credit card)
- Proof of current training in Cardiopulmonary Resuscitation (CPR), Obstetrical Emergency Skills (ES) and Neonatal Resuscitation (NRP)
- Proof of completion of the College’s Jurisprudence Course
- A complete Vulnerable Sector Check (effective April 1, 2019)
- Proof of ability to legally work in Canada – citizenship, residency, or work authorization
- Photo id
- A recent colour passport-sized photo
- Letters of Standing and Professional Conduct from all jurisdictions and regulatory bodies where you are currently or have been registered or licensed to practice any profession.
- Any name change information
- Post-dated cheques to cover registration fees
Applicants can submit the documents listed above along with payment through one of two methods:
- Cheque and mail. The cheque and form can be mailed to the College’s offices at: 21 St. Clair Avenue East, Suite 303, Toronto, Ontario, M4T 1L9. Cheques should be payable to the College of Midwives of Ontario and not combined with any other payment.
- Electronic submission and credit card. On the form, select that you prefer to pay by credit card. If you select this option at the time of registration you will pay the fee by credit card. Submit the form through the College’s secure deposit box form below.
Applications are reviewed to determine completeness. Once your application has been reviewed, you will be contacted and informed of any missing information or documentation by email.
Once all available items are received, the Registrar will refer your application to the Registration Committee. Applicants will be sent a written notice of referral outlining the reasons for the referral and providing an opportunity to make written submissions to the panel within 30 days of receiving the notice.
If you are a former registrant who resigned from the College within the previous five years and meets the requirements outlined in the section on former registrants returning to practise, the Registrar will refer your application to the Registration Committee for approval of a requalification program.
Former registrants who do not meet these criteria, e.g., it has been more than five years since you were registered with the College, the Registrar will review your application to determine that all requirements for a general certificate of registration are met and where any requirement is not met the Registrar will refer the application to the Registration Committee based on one or more of the following grounds:
The Registrar:
- has doubts, on reasonable grounds, about whether the applicant fulfils the registration requirements;
- is of the opinion that terms, conditions or limitations should be imposed on a certificate of registration of the applicant and the applicant is an individual described in subsection 22.18 (1);
- is of the opinion that terms, conditions, or limitations should be imposed on a certificate of registration of the applicant and the applicant does not consent to the imposition; or
- proposes to refuse the application.
The panel of the Registration Committee can then make one or more of the following orders:
- Direct the Registrar to issue a certificate of registration.
- Direct the Registrar to issue a certificate of registration if the applicant successfully completes examinations set or approved by the panel.
- Direct the Registrar to issue a certificate of registration if the applicant successfully completes additional training specified by the panel.
- Direct the Registrar to impose specified terms, conditions, and limitations on a certificate of registration of the applicant and specifying a limitation on the applicant’s right to apply under subsection 19 (1).
- Direct the Registrar to refuse to issue a certificate of registration.
Once you have received a written decision from the Registration Committee panel, you must successfully complete all requirements ordered by the Committee to become eligible for a certificate of registration.
Once you have successfully completed all requirements and are eligible for a certificate of registration, the College will verify that you have arranged liability insurance and that it will be effective as of the anticipated date of registration.
You must then pay all required registration fees in accordance with the College’s Fees and Remuneration By-Law.
Upon receipt of insurance and payment of fees, the College will issue the registration number and process the registration documents. The College will inform you of your registration, and the effective date of registration by email.
Once you have received confirmation from the College of registration, you may begin providing midwifery services. A wallet card and an electronic certificate of registration is immediately available via the registrant portal.
